Advance Health Care Directive

This document appoints a Health Care Agent (and a successor Agent) who will have the authority to make decisions on your behalf in the event you are unable to. You are always in control of your health care decisions until you are no longer able to make decisions for yourself. Your Directive will only become operative when two (2) physicians have determined that you are unable to make health care decisions on your own. This document is a "consensus-building" document that lets the Health Care Agent and your family members know your desires if you are faced with an end-stage condition.

It is often difficult for individuals to think about the care and treatment they want in the event they are incapable of making their own health care decisions. That's why we emphasize that this document is not for the individual: it's for the individual's family. As difficult as end of life decisions can be, it is even more difficult for family members to be faced with these decisions without any guidance. This document provides that guidance.

An AHCD allows individuals to appoint an agent who has power of attorney to make care and treatment decisions on their behalf, and give instructions about their health care wishes.
